The Carers Hwb at Swansea Carers Centre is the gateway to accessing help for you as a carer.

​​​Whether you are new to Swansea Carers Centre or have accessed our services previously, you can arrange a 1-1 appointment with us to discuss your caring role.

 

​What happens when you, as a Carer, contacts the Hwb for the first time?​

​​​​​​​We will use this appointment to find out how we can support you here at Swansea Carers Centre, and also make referrals to partner organisations on your behalf.

​

• You can phone our office on any weekday and an appointment will be arranged for you with a member of our team.

• We will use this time to explore any challenges that you are facing and provide you with easy access to information and advice.

• We will create a support plan that is tailored to your needs.

• We will follow up with you to review the progress of your support plan.

• You will receive ongoing support and will be able to continue to access all of our services.
