What is the Carers HWB?
The Carers Hwb at Swansea Carers Centre is the gateway to accessing help for you as a carer.
Whether you are new to Swansea Carers Centre or have accessed our services previously, you can arrange a 1-1 appointment with us to discuss your caring role.
What happens when you, as a Carer, contacts the Hwb for the first time?
We will use this appointment to find out how we can support you here at Swansea Carers Centre, and also make referrals to partner organisations on your behalf.

You can phone our office on any weekday and an appointment will be arranged for you with a member of our team.

We will use this time to explore any challenges that you are facing and provide you with easy access to information and advice.

We will create a support plan that is tailored to your needs.

We will follow up with you to review the progress of your support plan.

You will receive ongoing support and will be able to continue to access all of our services.
