What is Transition?
Transition is the process by which young people with a disability or additional needs transfer from Child to Adult Services in education, health and social care.
The transition from childhood to adulthood is a crucial time in the life of a young person with a disability or additional needs. The Dyma Ni project, funded by the Big Lottery Fund, will provide the right support to parent carers of young people (14+-25) going through this process to make it as easy as possible.
The project will:
provide information and training sessions on all aspects of the process of transition
support you to engage in the planning and delivery of transition and young adult services
create a peer advocacy programme to support you through transition
develop parent carer participation groups
